Surgery is appropriate for a genuinely selected minority — those with specific structural or neurological problems shown to benefit from surgical correction — not as a default response to persistent pain.

First-Line Non-Surgical Management

When Is Back Surgery Necessary?

Real risks and considerations include adjacent-segment degeneration (increased stress on the spinal levels next to the fusion over time), nonunion/pseudarthrosis (the bone fails to properly fuse), hardware-related complications, and the possibility of needing revision surgery later.

Microdiscectomy

This procedure removes the portion of a herniated disc that’s compressing a nerve root, and is generally aimed at radicular symptoms from nerve compression, not nonspecific axial back pain — this distinction matters enormously for whether it’s likely to help your specific presentation. It has reasonably good evidence for appropriately selected patients with genuine, imaging-and-clinically-correlated nerve compression, though recurrent herniation and persistent symptoms can occur, and it does not address disc degeneration more broadly, only the specific herniated fragment compressing the nerve.

Spinal Decompression Surgery
